"Fold all" does not fold all

Description

I have the XML/Stream.pm of XML::Stream opened. When I click "Fold All" it folds some of the subs but stops in the middle.
Once it stopped at row 1860 after unfold and "fold all" again it stopped at 2262.

I don't seem to be able to put my hand on some rules.

The "+" icon seems to be missing (Strawberry, Padre 0.86) while the "-" icon is shown.

Strawberry 0.86, confirmed on Ubuntu trunk r15115.
