Extract subroutined does not work if there is no subroutine in the code already

as I cannot select the name of the sub *before* what the new sub should be placed. r8744

OK, after much reading on PPI, I have a functional PPI implementation to find the right place to locate the new sub routine.

Needs lots of clean up in the code and then I'll commit it.
